Case report

We present the case of a 90-year-old man with a medical history of high blood pressure, hypercholesterolaemia and a pacemaker for third-degree atrioventricular block who attended Accident & Emergency with a two-day history of fever of up to 38 °C associated with oedema in the lower limbs and dyspnoea on minimal exertion, with orthopnoea and paroxysmal nocturnal dyspnoea.
