Variations in the opinion of breast surgeons and radiation oncologist regarding indications for radiation therapy after NSM: The need for prospective studies

Nipple-sparing mastectomy (NSM) has become a preferred option for many breast cancer patients, as it seems to be oncologically safe and may allow for a superior cosmetic outcome and higher levels of patient satisfaction.
There are no widely accepted indications for NSM, and as any new surgical techniques it depends on a learning curve, a curve that can vary for each technique and for each surgeon.
Over time and depending on surgeon experience, the indications are expanding, as well as the complications decline. One may question why there is not already a randomized trial comparing the NSM with other mastectomy types, the issue is that nowadays this not feasible or ethical, so reports on the oncologically safe and outcomes come mainly from retrospective studies.
