Primary cytoreductive surgery is considered as one of the cornerstones in the management of patients with advanced ovarian cancer. Recently, the CA-125 serum marker has been proposed for predicting optimal cytoreduction in advanced epithelial ovarian cancer.

In this study we assessed the role of preoperative CA-125 to predict optimal cytoreduction in patients with advanced ovarian cancer. Data on patients 67 patients (mean age: 57 years old, range 21–85 years) with advanced epithelial ovarian cancer that underwent primary cytoreduction surgery at our institution were reviewed. Optimal cytoreduction was defined as the largest volume tumoral residual disease of less than 1 cm. Optimal cytoreduction was achieved in 32 patients.
