Reconstruction of the anterior cheek, upper nasal ala, and lateral nasal sidewall

A 67-year-old woman with a history of nonmelanoma skin cancers of the head presented for treatment of basal cell carcinoma of the cheek and lateral nasal sidewall.

Given the size, location, and ill-defined margins of the tumor, Mohs micrographic surgical excision was indicated. The first-step margins were evaluated according to the Breuninger guidelines (slow Mohs), with paraffin-embedded pieces and staining with hematoxylin and eosin.

The final defect was geometric and measured 7.5 × 5.0 cm in size at the greatest diameter . How would you repair this defect?
